Call your insurance provider to verify that Dr. Amber Machin is listed, and to change your child’s primary care provider to her if necessary.  See our list of INSURANCE PLANS we accept.  We also see families without insurance, and offer a discounted rate on services provided if paid at the time of service.

Complete a MEDICAL RECORDS RELEASE FORM and submit it to your child’s previous primary care provider. This is extremely important for children with any medical problems which have required ongoing prescription medications, past hospitalizations, or specialist referrals.
